Today, we unveiled the 2024 Most Worth It Ranking—a compilation of the 20 highest-rated cosmetic treatments for the year, as determined by the Worth It Ratings from Ruli members. In our ongoing Most Worth It series, we are highlighting the trends emerging from this year’s ranking and the procedures to keep an eye on, helping you make informed, confident choices.
Sleeve gastrectomy tops the Most Worth It list
With a perfect 100% Worth It Rating (derived from 63 reviews by Ruli members over the past year), sleeve gastrectomy takes the top spot in this year’s ranking. This surgical procedure reduces the stomach’s size by 85–90%, drastically limiting the amount of food a patient can consume and generally leading to significant weight loss.
Numerous medical professionals recommend sleeve gastrectomy (also known as gastric sleeve surgery) over gastric bypass due to its less invasive nature; it boasts a lower complication rate and shorter recovery time; and unlike gastric bypass, it does not require lifelong avoidance of NSAIDs or dependence on nutritional supplements.
One Ruli community member shared that sleeve gastrectomy “truly saved and transformed my life.” Another patient reported that the procedure enabled her to become more active, leading to increased endorphins through exercise, which ultimately made her feel healthier, “both mentally and physically.”
Breast surgeries continue to be popular
Six breast-related surgeries made it into the top 20. Breast implant removal and breast reduction outranked breast implants, augmentation, and reconstruction, suggesting a shift towards smaller, more natural-looking breasts.
Dr. Melissa Doft, a board-certified plastic surgeon in New York City, links the rise in breast reductions in her practice to COVID-19 and stay-at-home orders. “It could be due to COVID-19-related weight gain, but I also think women were at home and realized they were unhappy with their shape and finally had the time to seek a solution,” she notes. “I’m also seeing a trend of women generally wanting smaller breasts. Before COVID-19, most women wanted a C or D cup—now they prefer a B or C cup. I believe this is because they are spending more time at home and wish to go without a bra, thus opting for a smaller cup size.”
Men’s top picks: gynecomastia surgery and hair restoration
Gynecomastia surgery, also known as male breast reduction, along with two hair transplant procedures (more commonly performed on men), received Most Worth It distinctions this year. Many Ruli members who underwent these surgeries described their results as “life-changing” and reported newfound confidence.
One reviewer of gynecomastia surgery wrote, “I was hesitant to wear T-shirts or go to the beach/swimming pool. I am extremely pleased with the results of my surgery and how my chest looks now. The surgery has greatly benefited me mentally by boosting my confidence and self-esteem.”
Wrinkle-reducing favorites: Dysport and Botox
Two neurotoxins commonly used to smooth fine lines, wrinkles, and frown lines—Dysport (96% Worth It Rating) and Botox (94% Worth It Rating)—surpassed all other injectable options.
While both are similar, Dysport has a lower molecular mass than Botox, giving it a higher diffusion rate. For this reason, some doctors find it more suitable for larger areas, such as the forehead, while Botox is ideal for more concentrated regions, like crow’s-feet. Anecdotal evidence suggests Dysport may take effect a few days earlier than Botox, which usually takes between 7 and 14 days. It’s important to note that everyone metabolizes different forms of botulinum toxin differently, so what lasts the longest for others might not be your best option. Methodology
To qualify as a 2024 Most Worth It procedure, a treatment must receive over 50 reviews from Ruli members between January 1, 2023, and December 31, 2023, and achieve a minimum cumulative Worth It Rating of at least 90% during that period. A total of 20 procedures (a mix of nonsurgical and surgical) earned the 2024 Most Worth It designation. While Worth It Ratings in the 2024 Most Worth It Ranking are presented as rounded percentages, the actual ranking is calculated to several decimal places.
